“After my father died, I put my hand on his grave, and said nobody else is going through this alone.”
— Pat Irwin (Founder at Eldercare Canada)
Pat M. Irwin
Founder & President, ElderCareCanada
I bring a deeply personal approach to elder care. After losing my father in 1997 due to inadequate care options, I left a 25-year career on Bay Street to channel that loss into founding ElderCareCanada — vowing that no family would face the same experience if I was by their side.
That mission is at the heart of everything I do. Drawing on my Certified Senior Advisor designation, mediation training, and consulting background, I work compassionately with families to navigate the emotional and practical challenges of aging — covering in-home care, housing, long-term planning, and complex family mediation — always with your loved one's well being at heart.
Mum’s in the hospital, Dad has dementia, and you live 5 hours away. Is this you? We can help.

I work with the entire family system. A core part of my approach is ensuring that the senior's voice, dignity, and wishes are respected and integrated into the plan, while also addressing the practical realities and concerns of the adult children.

This is incredibly common, and it is exactly why I specialize in family dynamics. Through eldercare mediation, I provide a safe, structured, and neutral environment to help families communicate effectively. We focus on bridging the gap between siblings and seniors so you can make unified decisions without damaging your relationships.
-
Family meetings about aging parents are often emotionally charged and can easily get derailed by past family history. Eldercare mediation brings in a trained, objective professional to guide the conversation. It ensures everyone—especially the senior—is heard, keeps the focus on practical solutions, and prevents conflicts from escalating into costly legal battles.
-
While I do not provide direct legal or financial advice, I help families develop the strategies needed to get these vital tasks done. I can guide your family through the planning stages for long-term care costs, estates, wills, and even funeral arrangements, ensuring you are fully prepared before you sit down with a lawyer or financial planner.
-
Yes. In addition to mediation, I provide expert opinions for families who are already undergoing formal mediation or litigation regarding eldercare costs and the distribution of caregiving effort. I can provide the objective, professional insights needed to help resolve these disputes.
-
Absolutely. Beyond just creating a strategy, I assist with the practical legwork. I can help you evaluate in-home care agencies, explore independent or assisted living housing options, and even provide hands-on relocation assistance when it is time to move.
-
Because I specialize in family dynamics, I know that what works for one family might not work for another. I draw on my background in consulting and service-quality training to deliver a deeply personalized experience. Before creating any strategies, I take the time to learn your family’s history, communication styles, and values. Whether your family needs a gentle mediator to navigate old sibling rivalries or a decisive project manager to handle absentee care, I adapt my approach to fit exactly what your family needs to feel supported, respected, and confident in your decisions.

Every family's journey is different, but the process begins with a comprehensive, personalized assessment of your current situation. First, we will sit down to understand your aging loved one's needs, housing environment, and wishes, alongside the concerns of the adult children. Next, I help facilitate a structured conversation so the family can plan together. Finally, I provide a customized, step-by-step action plan—and if you need it, I stay actively involved to help you implement the logistics, from hiring in-home care to managing a relocation.

Getting started is as simple as reaching out for an initial phone call. We will briefly discuss your family's current situation, the challenges you are facing, and what you hope to achieve. From there, we can schedule a full consultation to begin mapping out a customized strategy for your family.
